Form Description: This is a form to start a Divorce if the couple agrees on everything and has no children under 18, in high school or disabled. The husband and wife must provide various information on themselves such as where they currently live, when they separated, any orders of protection and injunctions filed by one spouse against the other, and any restraining order filed by a spouse against the other spouse. The divorcing couple must also supply financial information such as any real property, personal property, debts and if one spouse is paying the other spouse alimony.
