Form Description: This is a form that is used to determine who gets custody rights and visitation rights of the child(ren). An individual can request that the court not disclose their current address or the child(ren)'s address to protect them. Information needed for this form includes when and where the child(ren) lived in the last five years, whom they lived with, and what the relationship is they have with that/those person(s). The Court also wants to know if any custody, civil or criminal cases have been filed.
