Form Description: This is a form used to find out the current assets and debts of both the husband and the wife. Any information on any real estate owned by either or both the husband and wife must be provided and any other assets must also be disclosed. Either spouse can also make a property claim if they believe that the piece of property was a gift specifically to them and them alone. The couple must also disclose all debts and if they have gone bankrupt as well as what type of bankruptcy they filed for.
